Venetoclax (Venclexta): The BCL-2 Inhibitor and Its Expanding Role

Venetoclax, sold as Venclexta, is an oral inhibitor of BCL-2, a protein that helps cancer cells evade apoptosis. It was a landmark for targeting a pathway once considered undruggable and has become a core component of leukemia treatment.

Mechanism at a glance

Venetoclax is a BH3-mimetic that binds BCL-2 and releases pro-death proteins, restoring the cancer cell’s ability to undergo apoptosis. Because rapid tumor killing can cause tumor lysis syndrome, treatment starts with a gradual dose ramp-up and monitoring. Neutropenia is a common adverse effect.

Where it is used

  • Chronic lymphocytic leukemia: in combination with an anti-CD20 antibody (MURANO) and, since February 2026 in the US, with acalabrutinib in previously untreated disease (AMPLIFY), supporting time-limited therapy.
  • Acute myeloid leukemia: with azacitidine or other hypomethylating agents for patients not suited to intensive chemotherapy (VIALE-A).
  • Other settings: lymphomas are under study. In higher-risk myelodysplastic syndromes, the Phase 3 VERONA trial of venetoclax plus azacitidine did not meet its overall survival endpoint in 2025.

Competitive dynamics

In CLL, the central debate is continuous BTK inhibition versus fixed-duration approaches that include venetoclax. In AML, venetoclax-based regimens compete with intensive chemotherapy, new targeted combinations (for example menin inhibitors and FLT3 inhibitors), and cell-based strategies. Next-generation BCL-2 inhibitors are now reaching the market: sonrotoclax received accelerated FDA approval in relapsed mantle cell lymphoma in May 2026, and lisaftoclax received conditional approval in China in July 2025.

What CI teams should track

  1. Direct comparisons of fixed-duration regimens with continuous BTK inhibitor therapy.
  2. Resistance mechanisms in AML and the combinations designed to overcome them.
  3. Cytopenia management and the real-world feasibility of outpatient ramp-up.
  4. Emerging BCL-2 and MCL-1 programs.

Recent developments

Updated October 8, 2026. Items are drawn from company announcements, regulators and trade press; links go to the sources.

  • May 13, 2026: FDA granted accelerated approval to sonrotoclax (Beqalzi, BeOne), a next-generation BCL-2 inhibitor, for relapsed or refractory mantle cell lymphoma after at least two lines including a BTK inhibitor (response rate 52%, median duration of response 15.8 months). Venetoclax is not approved in mantle cell lymphoma, so this is a new lymphoma competitor, not a label overlap. (AJMC)
  • Apr 13, 2026: Lilly announced that BRUIN CLL-322 met its PFS endpoint: pirtobrutinib plus venetoclax-rituximab vs venetoclax-rituximab in relapsed or refractory CLL/SLL (639 patients). No numbers were disclosed. (ChemXplore)
  • Feb 19, 2026: FDA approved acalabrutinib plus venetoclax for previously untreated CLL/SLL (AMPLIFY): PFS HR 0.65 vs FCR or BR, with 14 cycles of acalabrutinib and 12 of venetoclax. The population excluded del(17p) and TP53 mutations. AbbVie announced it on February 20. (FDA)
  • Dec 2025: At ASH, CLL17 showed fixed-duration venetoclax-obinutuzumab and venetoclax-ibrutinib noninferior to continuous ibrutinib for three-year PFS in untreated CLL (909 patients; PFS HR 0.87 and 0.84). The investigators said longer follow-up is needed. (OncLive)
  • Jul 10, 2025: Ascentage announced conditional approval in China of lisaftoclax for CLL/SLL after prior therapy including a BTK inhibitor. The company describes it as the first BCL-2 inhibitor with conditional approval for CLL/SLL in China. (Ascentage filing)
  • Jun 16, 2025: AbbVie announced that the Phase 3 VERONA trial of venetoclax plus azacitidine in newly diagnosed higher-risk MDS did not meet its overall survival endpoint (HR 0.908). It said the data do not affect approved indications. (AbbVie via Placera)
  • Jun 2025: A Phase 1 study in the Journal of Clinical Oncology of azacitidine, venetoclax and revumenib in 43 older patients with newly diagnosed NPM1-mutated or KMT2A-rearranged AML reported a 67.4% complete remission rate. Differentiation syndrome occurred in 19% and QTc prolongation in 44%. (PubMed)

Upcoming catalysts

  • Dec 12-15, 2026: ASH Annual Meeting, New Orleans.
  • Ongoing: BeOne’s Phase 3 CELESTIAL-TNCLL compares sonrotoclax plus zanubrutinib with venetoclax plus obinutuzumab in untreated CLL; no readout date has been identified.
  • Pending: full BRUIN CLL-322 data, which Lilly says will be presented at a future medical meeting.

Bottom line

Venetoclax is a good example of a drug whose value comes from the partner it is paired with. CI teams should look at combinations and sequencing, since the same molecule competes differently in CLL than it does in AML.
